The Outreach Coordinator, Rona Mackinnon, provides support to people in their own homes and local communities. Rona supports people to plan and organise their lives, helping with person-centred planning, advocacy, advice and general support.
People using the outreach service can get support with housing, care packages, getting out and about in the local community, socialising, health visits, social work visits, benefits issues, communication and practical independent living skills. The focus of the outreach service is on improving, educating and sustaining both physical and mental health. Rona does this by working closely with medical professionals, families, social workers and other voluntary organisations to ensure a high quality and consistent approach.
